  1. Checks must be made out to Fayetteville Lions Club.

  2. Mail your competed form and check to Fayetteville Lions Club, PO Box 159, Fayetteville, TX 78940.

  3. If your registration will be postmarked by September 12, 2026, your check should be for $25 (early registration rate). If after September 12, make your check out for $35.
