  1. Checks must be made out to Fayetteville Lions Club.

  2. If your registration will be postmarked on or before September 12, 2026, your check should be for $25 (early registration rate). If after September 12, make your check out for $35.

  3. Mail your completed form and check to Fayetteville Lions Club, PO Box 159, Fayetteville, TX 78940.
